To our patients:
You are responsible for knowing your dental policy. For example, you will be responsible for any charges if any of the following apply:
-
Your dental plan requires prior authorization before receiving services at our dental office and have not obtained an authorization.
-
Your dental plan determines that the services that we rendered are not dentally necessary and/or not covered by your insurance plan.
-
Your dental plan coverage has lapsed or expired at the time you receive services at our dental practice
If you are not familiar with your plan coverage, we recommend you contact your dental carrier or plan provider directly.
Thank You,
Dr. Michael R. Gabor, DMD, PC and Dr. Soyna Kapoor, DDS
I understand and agree to these terms.
